Post Placement Reports

The CCAA requires adopters to complete two post placement reports on their adopted child. The first report should be completed within six months of the completion of the adoption order in China and the second report within six months of the first report. The report should include:

  • The name of the adopted child. this includes the original Chinese name and the present name if different;
  • The child's date of birth;
  • The name of the welfare institute in China which placed the child for adoption;
  • The health status of the child at the time of the adoption;
  • Your name and address;
  • The name of the local authority or approved agency which recommended you as an adopter;
  • The date you arrived in China to complete the adoption process and the document number of the "Notice to Travel to China for Adoption";
  • The latest information on the child. this should include:
    - The child's health status and what, if any, medical treatment the child has received in the UK;
    - Intellectual development;
    - How the child is settling and adapting to the family and community;
    - Whether the child is attending an educational establishment, e.g. playgroup or school;
    - Whether an application for British Citizenship has been applied for/granted.
  • Two or more recent photographs of the child. Audio and video tapes may also be included.

The post placement reports must be translated into Chinese as the CCAA will not accept it without a translation.

Both the English and Chinese version of the post placement report should be sent to the Depatment of Health who will forward them to the CCAA.

Useful hints, tips and information (Webmaster)

 

  • Keep copies of absolutely everything!
  • Get organised and keep a well structured filing system
  • Keep a record of all the dates when papers were sent off and to where etc.. If possible, phone to make sure it has arrived. Get the names of the people you speak to.
  • My experience is that the DOH in London is well organised. You will be advised of the procedure when they receive your papers. They will also inform you at each stage of the process and tell you how long it will take to reach the following stage. You will also receive a copy of the letter that goes with your application throughout the process until it reaches China. You will therefore find that as you are so well informed you may not need to contact the DOH for an update. If you do need to contact them however you will be restricted to certain times.
